Considering that most chili has beans in it (the musical fruit), it could be referring to the cook’s exposure to methane.
What makes me wonder, though, is the use of the inverted exclamation mark. In written Spanish, that’s used at the beginning of a sentence to show that the sentence is an exclamation rather than a statement. Is Ziggy beginning to reach out to the Latino community or is the cartoonist trying to subtly hint that the kitchen-staff is part of the underground economy of illegal immigrants? Since the cartoonist seems to only have the one shade of skin tone (pasty, I think is the proper term), it’s hard to tell. If so, ¡Viva la Ziggy!

Well, the thought I had is that most people’s doctors tell them to stop eating certain foods that are bad for their health. But this cook’s chili is so awful, his doctor wants him to stop making it for the sake of everyone’s health!

I thought this one was obvious. His doctor told him “Stay away from foods like chili.” He took it literally and now can’t even cook it because of what his doctor said.

Wait … Maybe the cook is making the chili, like from a bodily function somehow. Nah, I think that’s a stretch. I just wanted to say it. I actually think it means the chili is so bad that it makes him unhealthy just by being near it while cooking it.
